The goals of treating diabetic neuropathy are to prevent progression and reduce the symptoms of the disease. Tight control of glucose is important to prevent progression. Treatment involves control of hyperglycemia to improve symptoms and prevent complications while minimizing hypoglycemic episodes. Goals for treatment are maintenance of plasma glucose between 80 and 120 mg/dL (4.4 and 6.7 mmol/L) during the day and between 100 and 140 mg/dL (5.6 and 7.8 mmol/L) at bedtime (as determined by home monitoring and maintenance of HbA1c levels < 7%. Analgesics (pain medications) may work for some patients on a short-term basis. But, in most cases, they usually do not provide much benefit.

Tramadol, a leg relaxer, and a level, NO yo-yo blood sugar is the thing I do. Tramadol and a Leg relaxer, it's Mirapex. 125MG, 1/2 hour before sleep was the final solution so I can finally sleep and think of other things beside the pain in my legs and feet. Any ways check with your Doctor and find out what he'll prescribe for your relief. Do as instructed and hope for the best. Remember to log, day to day your blood sugars and be sure to include medicines you are prescribed to see if they are going to work well and efficient with your Diabetes.

I have peripheral neuopathy in both lega and arms but not due to diabetes and I was miserable until I found a good Pain Specialist. I was on the same meds you listed and nothing finally i was put on time release pain meds and they help much better than the short term ones like vicodin. Most pain spec. have a history in anesthesia and are licensed to precribe different meds than doc's that do not deal with chronic pain. Do not settle for living with the pain. I am in Illinois near Chicago and have an awesome doc but their are Pain specialists usually at all major hospitals. Feel free to contact me if you need help finding one in your area as I have a ton of resources, just click on my picture. I hope this helps!

I have peripheral neuropathy in all four extremities. I was given Gabapentin. This helps a lot. However, I also take Oxycodone and Tramadol for pain, and this definitely works. The only thing is that with Oxycodone, it is a narcotic, and can cause addiction. Your doctor can answer these questions.

Many people find that Benfotiamine (A fat soluble synthetic form of vitamin B1) helps. Omega 3 EFA and Alpha Lipoic Acid supplements have also shown promise, as they are anti-inflammatory. You can find all of these at your health food store or online. I suggest these as none of the current drugs do much except to mask the symptoms of neuropathy for patients, and many have undesired side effects. Some, as you know, have little to no benefit.
I would give it a try. Also remember to try and keep your blood glucose levels in range and stable as much as possible, as this will improve your symptoms. This is VERY crucial.
To be honest, from what I have seen, our current ability to treat neuropathy is quite dismal using traditional Rx medications.
You can also try alternative therapies such as acupuncture or biofeedback.
It is a very tough to treat condition.
Do you have autoimmune Type 1 diabetes? If so, there might be autoimmunity towards your nerves involved here. If you do have Type 1, please tell me so I can elaborate on additional treatment options.
